(This form is not applicable to Midstream Heights, Ascend to Midstream & Retire@Midstream)

B. CANCELLATION OF STANDBY ELECTRICITY
Notwithstanding anything contained in, and without prejudice to the MES "terms and conditions", the consumer and / or MES may cancel the contract after six (6) months from the first date of connection.
C. AGREEMENT TO THE TERMS AND CONDITIONS (https://mes.midstream.co.za)
I declare that the abovementioned information is correct and accept that the supply of the service will be in accordance with the “Terms and Conditions for the supply of electricity by MES”. MES is committed to the lawful collection, use, management of, processing and storage of personal data in terms of the Protection of Personal Information Act, Act 4 of 2013 (“the POPI Act”) and recognises that everyone in South Africa has the right to privacy, as provided for in Section 14 of our Constitution. MES only collects information for the effective administration of the relevant account and that no information is gathered for or will be disseminated to third parties for commercial or marketing purposes.
D. GENERAL NOTES
1. No upfront fee is payable at application. The cost for standby electricity during periods of loadshedding will be a surcharge – thus a cost based on an averaged tariff, over and above your normal tariff;
2. The surcharge cost shall be based on the units used by you as the customer / consumer at your residence during the period(s) of loadshedding multiplied by the averaged tariff. The averaged tariff as per the MES tariff sheet available on the internet;
3. The average rate of consumption (kW units consumed per hour) shall determine the averaged tariff, with a higher rate charged for those with a higher consumption rate. The average rate of consumption will be determined by totalling the units consumed during loadshedding over a 7-day period and dividing same by the duration (hours) of loadshedding over the same 7 day period. Information on units consumed and duration of loadshedding will be available on the customer profile on the internet.
E. STATEMENT BY CUSTOMER / CONSUMER:
